Eligibility Criteria

At EFry Health Services Society, we are dedicated to serving vulnerable women and children by addressing their comprehensive needs—medical, psychological, emotional, and social. To ensure our support reaches those most in need, each applicant must complete a detailed intake form. This allows us to assess their situation thoroughly and determine eligibility based on defined vulnerability criteria.

A multidisciplinary team—including intake officers, nurses, and administrative staff—reviews each case with care and professionalism. Through this process, we ensure our services are reserved for those facing genuine and urgent challenges, allowing us to provide focused and effective support.

   To be eligible for this program, both of the following conditions must be met:

        • The individual must be a resident of Surrey-North Delta B.C. OR a referred or existing client of EFry Greater Vancouver Programs and Services across the lower mainland.
        •          The individual must not currently have a family doctor or must be willing to change doctors.

Surrey-North Delta catchment area includes the following six communities:  North Delta – West Newton; North
Surrey-Whalley, Guildford, Cloverdale-Panorama, Fleetwood, and East Newton.

             AND

      Satisfy two or more of the following eligibility criteria:

        • Homelessness / Housing insecurity
        • Substance Use / Substance Use Disorder
        • Mental Health condition(s)
        •  Impacted by the Criminal Justice System
        •  Domestic Violence 

 

Upon confirmation of eligibility, the applicant will be contacted by reception to arrange an intake appointment—beginning with the nursing team, followed by the provider. It is imperative that the nursing assessment be conducted first, as it allows for a timely evaluation of the applicant’s immediate needs and informs the appropriate scheduling of the provider consultation.

The EFry Health Services Society team respectfully requests that referring organizations or individuals submit referrals only for patients who clearly meet the established eligibility criteria. This approach ensures a streamlined admission process and minimizes misunderstandings or unmet expectations on the part of the applicants.

If your patient/client does not meet all of the listed criteria, we encourage you to contact our intake team at (604) 282-3944 to discuss their situation.  While we strive to follow our intake guidelines, we are open to conversation and will consider each case thoughtfully.
