Eligibility Criteria
Women and children who meet the following criteria are supported at the EFry Health Center
1. A resident of Surrey-North Delta B.C. OR an existing client of EFry Greater Vancouver Programs and Services across the Lower Mainland.
AND
2. Do not have a family doctor in B.C.
AND
3. Individuals who are currently experiencing 2 or more of the following: Homelessness / Housing insecurity · Substance Use / Substance Use Disorder · Mental Health condition(s) · Domestic Violence and/or have been impacted by the Criminal Justice System
(The Catchment area for the Health Centre is Surrey-North Delta, which includes North Delta-West Newton; North Surrey-Whalley, Guildford, Cloverdale-Panorama, Fleetwood, and East Newton)

How to Become a Patient
1. Call us to inquire about eligibility and availability for you and your children.
2. Complete a short intake form for yourself and any children you may have under the age of 19 years.
3. Once the intake form (s) are filled in and returned to the Health Center you and your children will receive an appointment with our intake team.
Interpretation Services:
We are happy to arrange interpretation support. Please let us know your preferred language.

Appointments can be booked by phone during clinic hours.

Please call at least 24 hours in advance.
Late Arrival
We will do our best to accommodate you, but you may be asked to rebook.
